CASE MANAGER – LIGHTHOUSE COMMUNITY

The primary duty of the case manager is to empower residents with the skills, resources, Job Purpose:

Empower Residents with skills, resources and education so that they realize their internal power, overcome their addictive behavior and become independent and education so that they can realize their internal power, overcome their addictive behavior, and become independent individuals.  Specific duties are to provide case management to residents, develop individual treatment plans, enter resident’s  information in ROSIE data base upon admission, update resident’s progress in data base, maintain residents case files,  create and maintain relationships with feeders, knowledgeable of the program’s policies and procedures,

EDUCATION

 A degree from an accredited institution in addiction and working toward, the following certification levels: Trainee, CSC-AD, CAC-AD
